School starts in Roane County in a little more than two weeks, but students going into prekindergarten, kindergarten and seventh grade may not be admitted if they don’t have the proper immunizations.
The Tennessee Department of Health also requires an official certificate for entry into school for these students.
This form will only be available from a private health-care provider or health department.
